The DVA Community Nursing Program provides veterans, war widows and widowers (entitled persons) with access to a range of quality health care and related services, including home community nursing services.
Community nursing is a great form of care because it aims to enhance your independence and your lifestyle in a safe and familiar environment, thus enabling you to remain in your own home for longer. The Community Nursing Program is delivered by registered nurses, enrolled nurses, and nursing support staff. Kinds of care include clinical nursing care and personal care.
All those who have served including war widows with a gold card and some holders of white cards may also be eligible for funding by the government if they meet the criteria.
If you think you could benefit from Community Nursing services, you can contact your local doctor, or hospital doctor where you are receiving treatment, or the person managing your discharge from hospital.
We will arrange for nursing staff to come to your home to care for you.
If you are a General Practitioner (GP), Nurse Practitioner specialising in a community nursing field, Treating Doctor in Hospital or Hospital Discharge Planner, it is as simple as completing the online referral form to get the process started.
Aster Home Care will work with you, and your family member or care giver if needed, to create a nursing care plan.
This nursing care plan will detail the type of care you receive, such as:
You will be asked to sign a copy of your nursing care plan to show that you agree with it. You will be able to keep a copy for yourself. If you cannot sign it, your family member or carer can sign for you.
Over time your care needs may change. A nurse will review your nursing care plan regularly and update it if needed.
If you are receiving Community Nursing services, both you and your provider have rights and responsibilities.
We want you to be confident that you can give feedback about our services, or raise a concern, and know that it will be treated in a timely, fair, and transparent manner.
