Referrals
Aster Home Care is a health care provider that specialise in clinical nursing and home care. With over 25 years in the industry, we pride ourselves on exceptional holistic care.
Nursing Services provided by our nursing staff can include:
- Personal Care
- Caring for wounds
- Helping with medication
- Helping to manage symptoms of medical conditions
- Helping with end of life and palliative care
- Catheter care / Catheter change
- Meal prompt or assistance with meal preparations, (heating food, making beverages, setting up to eat).
- Referrals to an Occupational Therapist, Dietician, Physiotherapist or Social Worker
- Continence Management
- Helping after an operation or stay in hospital
- Helping with day-to-day activities, like dressing, showering or going to the toilet
- Helping with other tasks if there is a clinical need
Who can be referred to Aster Home Care:
- Veteran Gold Card holders
- Veteran White Card holders
Who can refer to Aster Home Care:
- General Practitioners (GP’s)
- Nurse Practitioners specialising in a community nursing field
- Treating Doctors in hospital
- Hospital Discharge Planners
How to refer a patient to Aster Home Care:
- Via HealthLink Connect, using our EDI:asterhmc
Referrers using Medical Director will need to search the MD exchange online directory for facility code “hmsweber”, copy this to their local address book and edit this to our details. In order for referrals to match, referrers will need to enter our EDI after AHC name in parenthesis: “Aster Home Care (asterhmc)” - You can complete a referral form. The referral form can be emailed to nursing@asterhomecare.com.au

- We will also accept a verbal referrals, providing we receive a written referral within 7 days.
